Talk About Cholesterol – to Reduce or not to Reduce High Cholesterol, Do We Need Low-Cholesterol Diet?

Fight with high cholesterol seems to have become a world-wide tradition recently. We are being asked to trust that cholesterol is the main cause of atherosclerosis and death by cardiovascular diseases.

Therefore, patients with high cholesterol are prescribed with low-cholesterol fat-free diet and are pumped with statins. Over many years pharmaceutical companies have been enriched by sale of statins, however, strange as it may seem, diet and statins failed to decrease rate of myocardial infarction and strokes as well as mortality caused thereby. Moreover, physicians are well aware that every second person with experience of myocardial infarction has a normal level of cholesterol. But mortality rates are two times higher among people with low cholesterol. What is this all about?

Constantly criticizing cholesterol like nothing else on earth, most patients have no idea that cholesterol is vitally important to the human organism. Unfortunately, many physicians forget about this too.

What kind of chemical compound is this and what is its function in the organism?

Cholesterol is a secondary mono alcohol, lipoid, that in its free form together with phospholipids makes part of cell membrane structure, providing stability thereof. Cells cannot function properly if their cell membrane is impaired.

Cholesterol is used as building material in generation of adrenal cortex hormones, male and female sex hormones, vitamin D. Liver uses cholesterol to generate biliary acids, which enter the intestinal tract, where they participate in fats assimilation.

A lot of people don’t know that cholesterol is also a type of an anti-oxidant and that it serves as a means of protection against cancer. 23% of cholesterol is stored in the brain, the central nervous system cannot function properly without it.

Reduction in cholesterol level causes sexual dysfunctions, memory disorders, Parkinson disease, suicidal ideation, aggression and, paradoxically, strokes. Cholesterol is required for proper functioning of immune system.

A little known fact is that approximately 80% of cholesterol is generated in the liver and just 20% of cholesterol is provided with food.

Lack of animal fats in food affects liver functioning. Getting no vitally important cholesterol with foodstuff, liver pools all available resources to generate its own cholesterol from available carbohydrates. Liver has to function under great stress, assuming the inappropriate function.

How does cholesterol move through the human organism? Neither fat, nor cholesterol can move in blood as free elements because fats are insolvable in water. For cholesterol to get from liver to cells, molecules thereof bind together with phospholipids and proteins. Such a structure is called lipoprotein. Lipoproteins could have different composition. If lipoproteins contain a small number of compacted cholesterol, such lipoproteins are called high-density lipoproteins (HDL). Such particles have high density and small size. If there is a large amount of cholesterol, which is covered with small number of phospholipids, such a structure has larger size and lower density. Such lipoproteins are called low-density lipoproteins (LDL). The above-mentioned types of lipoproteins have opposite functions. LDL, which is for some reason called bad cholesterol, transports cholesterol, generated in the liver, to cells. HDL transports cholesterol from cells back to the liver, which uses 80% of it to produce biliary acids. Both functions are equally important! These are the reasons why there is no bad cholesterol for the organism, even the bad cholesterol, or, scientifically speaking, low-density lipoproteins, performs functions, which are essential for the organism. Cholesterol itself is neither good, nor bad!

Cholesterol became infamous as a companion of atherosclerosis and therefore, heart attacks and strokes, a long time ago. But what is atherosclerosis? Atherosclerosis is a disease of blood vessels. With this disease builds-up of a kind – plaques appear on internal surface of arteries, while, normally, the surface of arteries should be smooth, flat and glossy. Such round or oval plaques have uneven surface and, naturally, protrude into the blood vessel cavity. This way the plaques decrease the artery diameter, thus hindering the blood flow therein. Well, cholesterol is the basis of such an atherosclerotic plaque. From this finding the scientists concluded that it is necessary to fight the enemy, i.e. cholesterol, however they overlooked many important factors. Normally, excessive cholesterol is disposed, i.e. it is returned back to the liver for reprocessing and the organism is able to control its generation depending on amount of cholesterol, supplied externally with food. But where are plaques come from then, why no one asked, why they appear, for no reasons at all or what? But no, cholesterol was announced enemy No 1 and that was all. However, many biochemists, physiologists, gerontologists do not agree with the cholesterol theory.

As far back as in the 19th c a well-known Viennese surgeon Theodor Billroth conducted interesting experiments in goats and sheep. He removed their thyroid glands, after that their cholesterol increased sharply and a typical atherosclerosis pattern developed. However, both goats and sheep are herbivore, they do not eat animal matter. Consequently, atherosclerosis was caused by lack of thyroid gland hormones.

It should also be noted that unlike men, women, generally, suffer from heart attacks and strokes at older age, during climax, when estrogens level decreases, i.e. low estrogens level also contributes to atherosclerosis development.

If arteries walls are not damaged, if they are smooth, even, glossy, there will never be cholesterol deposits thereon. Cholesterols deposits in the places, where arteries are damaged. Otherwise, a clot would appear on the damaged site. A large number of factors contribute to damage of arteries walls, e.g. high glucose and insulin level, smoking (nicotine is a factor, damaging the vessel wall), alcohol abuse. Studies, conducted i.a. by Russian specialists, have shown the role of herpetic viruses, causing inflammation of vessel walls. Autoimmune process can also be a damaging factor. Lately, still greater importance has been attached to high level of homocysteine amino acid, damaging vessel walls. Homocysteine level increases due to impairment of methionine essential amino acid metabolism, when there is insufficient level of vitamins B6, B12 and folic acid in the organism.

Thus, high cholesterol level is only a sign of inflammation in the organism, but not the cause thereof. Cholesterol is at hand to patch the holes in the organism. Cholesterol is a building material to repair damaged inner (!) vessel walls.

High cholesterol is related to the size of damaged vessels area. The more there are damaged vessels, the more cholesterol is required.

Here is one more curious fact. It has turned out that people, maintaining low-cholesterol diet, still have high cholesterol. The less animal fats the food contains, the higher cholesterol level and weight is. Improper treatment may have a sad outcome, i.e. another heart attack.

Is it really necessary to take statins, the more so as they have serious side effects? In some cases, statins might be really necessary, if there are generic causes of lipid metabolism impairment. However, the patient should agree upon this issue with his or her attending physician after detailed examination.

How to Deal with Back or Joints Pain?

As is well known our older generation is fond of spending their time at garden plots, where they do their best to have a good harvest. However, excessive physical exertion results in pain syndrome, which is the manifestation of osteochondrosis or arthrosis.

The spinal column is the main supporting osseous structure of the organism. It is comprised of vertebrae, which account for stabilization of body position. Intervertebral discs, lying between adjacent vertebrae, serve as the main shock absorbers in the spine and ensure movements among structural elements of the spine. On the outside they are covered with fibrous tissue, and there is vertebral pulp inside.  Such structure enables intervertebral discs to ensure even distribution of pressure on vertebrae.

The spinal cord undergoes substantial strain when a person runs, jumps, falls, etc. This is why we need elastic and resilient discs that alleviate pressure on vertebrae thus protecting the spinal cord from damage.

Osteochondrosis is a disease, causing deterioration of intervertebral cartilage discs structure and their gradual destruction. Generally, pathological changes are age-related. Amount of liquid in discs reduces, discs get drier and flatter. They also become less flexible and fail to provide absorption function.

When osteochondrosis progresses, disc surface becomes cracked, gelatinous substance starts to bulge out into the spinal canal. This condition is known as protrusion. If there is no treatment, outer coat breaks (fibrous ring) and the nucleus protrudes, which causes hernias of intervertebral discs.

But as usual a new season may bring new problems. One of them is the so called sun allergy. According to medical statistics, number of registered sun allergy cases increases every year.

People with blond hair, pale skin, young children, people with recent tattoo or those, who have undergone peeling procedure recently, pregnant women, men and women, using a solarium, are a group at risk.

Taking of medications is one of the factors, contributing to sun allergy occurrence. It may be manifested as photosensitivity to drugs when exposed to the sun or phototoxic action of the drug.

Use of Matrix Products for Office Syndrome

Advancements of information technologies, labour automation, physical inactivity and increasingly stereotyped movements of office workers have given rise to new diseases, related to impact of office environment. Generally, length of the working day in the office makes 8 hours, most part of which the worker sits at the computer at his or her workplace, which often does not conform to the existing standards with regard to ergonomic parameters thereof. Other risk factors, contributing to development of office syndrome, are also to be taken into consideration, i.e. large crowds of people, unhealthy diet, irregular working hours, large number of allergens, etc.

Migraine as well as tension headache belongs to primary headaches, i.e. headaches that are not caused by brain or cervical spine or any other disorders. The disease is most common in women from 18 to 50 years of age.

The underlying reason of migraine episodes is disorder in dura mater vascular tone regulation, i.e. vascular distention, increased vascular permeability and ingress of various biologically active substances, causing pain, into the surrounding tissue with the formation of neurogenic inflammation. Consequently, many patients are wrong to administer vasodilators, such as No-Spa to arrest an attack of the disease, such medications make only worse, since, contrary to popular belief, the cause of migraine attack is not spasm of blood vessels but their pathologic distention.

Spine, neck, upper extremities pain. Most frequently such a pain is caused by a long-term forced position of the body and refers to musculoskeletal pain.

Mouse arm syndrome (carpal tunnel syndrome). Obesity increases strain on the knee joint by two times.

Who is at Risk?

People in the Age Group of 50 Years and Older

Approximately 65% of all people, suffering from vision impairment, are people of 50 years and older, while this age group makes 20% of the world population. Population ageing is typical for many countries, therefore the number of people at risk of age-related vision impairment is increasing.

Children Before the Age of 15

It is estimated that 19 mln people suffer from vision impairment. 12 mln of them have vision impairment caused by refraction anomalies, the conditions, which can be easily diagnosed and corrected. 1.4 mln children are irreversibly blind to the end of their lives.

Global Measures to Prevent Blindness

Use of Health Matrix Products for Myopia

It may be argued that the first mention of myopia goes back to the 4th BC, in Aristotle works. He noticed that some people need to take objects close to their eyes to have a good look, besides, they often squint to see things.

Myopia is one of the most common vision disorders in the world. According to some sources, short-sightedness affects more than 1bln people. Essentially, myopia is a type of ametropy – pathologic change of the refractional function of the eye.

Normally, light, entering the pupil, is refracted by the main eye lens – crystalline lens and is projected to the retina of the eye. The retina is the structure of photosensitive cells, where the image is generated, i.e. outlines and color of objects.

When refraction is impaired, the image is projected not on the retina itself, but behind it (hypermetropia) or in front of it (myopia). If the focus point is in front of the retina, it impairs the ability of the eye to see objects at a distance as their projection is too far away from the retina, however, the eye sees close objects quite well. In case of high myopia vision can be impaired at close distances too.

The most common reason of myopia is an elongated eye-ball, as a result of which the retina turns out to be behind the focal plane. A less frequent variant is when the refracting eye system focuses light stronger than it is necessary and therefore the light focuses in front of the retina, not on the retina itself. In any of the above-mentioned variants a fuzzy, blurred image is formed on the retina when long-distance objects are viewed.

Over the last 10 years number of people with myopia has grown significantly. People wearing glasses have become an integral part of contemporary life: globally, approximately 1 bln people wear glasses. Myopia mostly affects young people. Thus, according to different authors, myopia frequency in schoolchildren ranges from 2.3% to 16.2% and more. Among students of higher educational institutions this percentage is even higher. Even though a hereditary factor plays quite an important role in myopia development, it is not necessarily a determining factor.

Factors, Contributing to Myopia Development

The following factors are to be considered as the ones, determining myopia development:

  1. Hereditary factor is of great importance as short-sighted parents often have short-sighted children. This is particularly evident in large groups of people. Thus, in Europe myopia incidence among students makes up to 15%, while in Japan – 85%.
  2. Adverse environment, in particular, prolonged work at a short distance. This is professional and school myopia, which develops particularly easily at the stage, when development of the organism is not finished yet.
  3. Primary weakness of accommodation, causing compensatory elongation of the eye-ball.
  4. Unbalanced tension of accommodation and convergence, causing spasm of accommodation and development of pseudomyopia, followed by actual myopia.

Despite relatively high level of ophthalmology, there is no sufficiently substantiated scientific concept of myopia development. Contribution of the above-mentioned facts should be deemed reasonably plausible, however there is no convincing evidence on precedence of any of them. Obviously, different types of myopia have different causes and myopia development stems from one of the factors or has a complex origin.

As it has already been mentioned, the cause of myopia is a changed form of the eye ball. It begins to look more like an oval than a circle. This results in impaired light refraction, therefore light goes through the eye ball and focuses opposite to the retina.

When accommodative ability is weakened, the eye does not cope with operations, demanding visual tension at short distances. In this cases the organism has to modify the optical system of the eyes so that to adjust it for work at a close distance without increased tension during accommodation. This is mostly achieved through moderate targeted elongation of the anterior-posterior axis of the eye during growth thereof and refraction formation. The result thereof is myopia, which may progress slightly, however, generally, it is no more than 3.0-4.0 diopter.

Two main pathogenic chains of myopia have been identified based on the studies of the Helmholtz Moscow Research Institute of Eye Diseases:

  1. Discrepancy between capacities of a weakened accommodative apparatus of the eye and visual workload.
  2.  Weakening of the sclera strength and elongation thereof under the action of intraocular pressure.

The first mechanism implies more favorable path of myopia development, in this case myopia is an optical impairment of the eye, the second mechanism implies myopia as a serious disease, prone to progression and complications. A hereditary factor plays an important role in development of both types of myopia. The first type is generally inherited as autosomal dominant, while the second type as autosomal recessive disorder.

Hereditary myopia manifests in girls somewhat earlier than in boys. This appears to be related to modifying gender influence and endocrine changes in organism at puberty.

The first signs of myopia appear at the age from 7 to 12 years and progress until 20 years in women and 22 years in men. Subsequently vision usually stabilizes, however it also may deteriorate further. When anterior-posterior axis of the eye increases, cells of retina, located in the area of maximum illumination sensitivity become sparse like the fabric of a stocking when pulled up the leg. This may cause development of other diseases. E.g. age-related macular dystrophy, generally, develops after 55 years, however in case of severe myopia it can develop at an earlier age.

It is common to refer to three myopia degrees:

Mild degree – up to 3.0 diopter

Moderate degree –  from 3.5 to 6.0 diopter

High degree – more than 6.0 diopter

With regard to clinical progression myopia is divided into the non-progressing and progressing myopia.

Myopia progression can be slow and may stop when organism growth stops. Sometimes, myopia progresses continuously and reaches high levels (up to 30.0-40.0 diopter), it can be accompanied with a number of complications and considerable vision impairment. Such myopia is called malign myopic disease. Non-progressing myopia is a refraction anomaly. It is manifested clinically as distance vision deterioration, it can be successfully corrected and requires no treatment. Temporarily progressing myopia has a favorable course too. Continuously progressing myopia is always a serious disease, which is the main cause of disability, associated with visual organ pathology.

Elongation of the posterior segment of the eye ball results in anatomical and physiological changes. Vision function is particularly affected by vascular and nervous tunics impairments, causing typical patterns of the fundus of the eye.

Elongation of eye layers is accompanied by increased vascular fragility with repeated hemorrhaging into the retina and vitreous body. Slowly dissolving hemorrhages cause opacity of vitreous body. Of particular importance is formation of a coarse pigmented area, significantly deteriorating vision acuity. Vision impairment may also be caused by progressing opacity of vitreous body, detachment thereof and development of cataract complications. Very serious complication of high myopia is a retinal detachment, developing due to breaking thereof at different sites of the fundus of eye.

Based on the above-mentioned it is clear that even mild and moderate myopia, that can be successfully corrected with optical lenses, may be an obstacle to choosing of a number of profession.

High continuously progressing myopia inevitably results in disablement due to sharp decrease of visual acuity or even complete loss thereof.

Use of Health Matrix Products for Prevention and Treatment of Occupational Diseases of Musicians

At the end of the 19th century hand diseases affected pianists so often that some musicians were forced to virtually review fundamentals of teaching methods. Nowadays this is still an urgent issue. Now we even have The Center for Musicians Medicine in Berlin, where occupational diseases of musicians, methods of prevention and treatment thereof are studied.

Prevailing types of occupational diseases manifestation among musicians can be referred to movement disorders in the form of abnormal state of motor system (impairment of normal, balanced functioning thereof during instrument playing, overexertion of muscular system of a kind). The above often results in involuntary muscle contractions, generally called convulsions, and neuralgic pains. Occupational neuralgias include, first of all, brachiplex neuralgias. The so-called neuromyalgia, neuromiositis, neurotendomyositis are also common.

A musician has to be able to operate his or her muscles, nerves and bones (locomotor and nervous systems) with great skill. Playing musical instruments requires controlled repeated movements and often implies staying in unnatural body position during many hours of rehearsals and performances. Such strains on the body may cause development of specific health disorders. Furthermore, poor working conditions: increased noise level, prolonged periods of play without breaks and preparation of a new complicated repertoire or use of new instruments can impact health of musicians regardless of their age and skill level.

Studies, conducted in the USA, Australia and Canada, have shown that approximately 60% of musicians get injuries, which can be detrimental for their profession, during the period of their professional activity. The studies have identified certain most common diseases: different overplay syndromes, including strain of muscles and connective tissues, controlling flexing and extending movements of hands. Such syndromes appear as a result of repeated strong movements of muscles and tendons. Pain in the back as a result of inconvenient position during the musical instrument playing is another widespread phenomenon among musicians. When playing the below-mentioned musical instruments, load is placed on nerves of the wrist and forearm, shoulders, arms and neck. Muscle spasms (focal dystonia) are also infrequent occupational disease of musicians. It may strike musicians in the prime of their career.

Bow instruments: violin, viola, cello, double bass, harp, classic guitar, electric guitar.

Health-related conditions of musicians, playing stringed instruments, are often subject to the way the musician holds the instrument and the way he or she sits or stands during the performance. E.g. most violin players or viola players put their instrument between the left shoulder and the chin. The musician often raises the left shoulder and lowers the left part of the chin and the jawbone so that the left arm could move freely along the neck. Simultaneous raising of one part of the body and lowering of the other part result in static muscle contracture condition, causing pain in the neck or the shoulder, temporomandibular disorders and thoracic outlet syndrome, when pain and numbness of the neck, shoulder and the upper chest are observed. Prolonged static sitting, in particular, in a bent position causes pain in the group of muscles, ensuring core stability. Playing the double bass, harp or classic guitar often requires static spine rotation. Heavy electric guitars often hang on a strap, worn over the shoulder at the left side of the neck. Therefore, there is a continuous pressure on the brachiplex, which may cause pain. Position-related problems are also one of the reasons of overexertion of wrist and fingers nerves and muscles as there appears a phenomenon of their pseudo-extending. E.g. the left wrist may be used for numerous repeated flexes, causing tension of extending wrist and fingers muscles and development of thoracic outlet syndrome. Pressure on nerves of the shoulder and the forearm can exacerbate disorders of the elbow, including the compression syndrome and neuropathology of the elbow bone.

Keyboard instruments: piano, organ, harpsichord, synthesizers and pianola.

Keyboard instruments playing requires a position similar to that of a typist. The head is often directed forward and down to see the keyboard and hands and repeated upward movements to see the score result in neurologic and muscle pain in the neck and back. A musician often rounds shoulders and raises his or her head up in addition to simultaneous shallow breathing. Continuous compression of nerves and blood vessels in the muscles of the neck, shoulders and ribs may result in development of a carpal tunnel syndrome. Moreover, tendency to flex wrists and bend fingers, keeping finger/wrist joints flat causes an excessive stress of wrist and finger muscles. Plus, due to continuous use of the thumb in the position lower than the hand, muscles overstrain, stretching and flexing fingers extensors. Efforts, undertaken to play large chords or octaves, may result in overexertion of the wrist joint capsule and ganglion development. This group of musicians frequently suffer from muscle spasms and focal dystonia. In this case sometimes it takes much time to learn correct hand positioning during neuromuscular training, which is needed to avoid further problems.

Wind and brass instruments: flute, clarinet, oboe, saxophone, bassoon, trumpet, French horn, trombone, bass horn and bagpipe.

A musician, playing one of this instruments, changes his or her position in such a way as to control the air flow as the position creates area for diaphragmal and intercostal breath blowing. Sound of the above-mentioned instruments depends on the way the mouthpiece (embouchure) is held, which is controlled by facial and throat muscles. The way the mouthpiece is held also influences sounds, generated by vibrating body or the mouthpiece. The position also influences the way a musician holds the instrument while sitting or standing and pressing the buttons of the instrument, controlling the pitch of the tones. Thus, French open hole flute requires stable position and bending (forward) of the left shoulder, stable position (backwards) of the right shoulder and slight turn of the head and the neck to the left. The left wrist is often held in an extremely bent position, while the arm is stretched so that to support the instrument by the crooked index finger and by two thumbs, balance is kept by the right little finger. This results in heightened tension of wrist muscles and muscles that flex and extend fingers. The habit to stretch the head out and breath shallowly increases the risk of thoracic outlet development.

Percussion instruments: drums, tympanum, hammer dulcimer, marimba, table and taiko.

Use of sticks, hammers and bare hands to tap the instruments results in hands and fingers injuries in no time. Pulse vibration, caused by taps on the instrument, is transmitted upwards via the arm and may cause injuries of tendons and peripheral nerves. Such biomechanical factors as the level of the applied force, repeated play and static load on muscles may also cause injuries. This group of musicians is frequently affected by the carpal tunnel syndrome and tendon nodules.

  1. Main Cause of Any Occupational Disease

The main cause of any occupational disease is loss of the integrity, unity, harmony of the performing musician’s motor system, essentially involving all of his/her body, from the finger tips to tips of the toes.

Natural balance of all components of the system is upset: it is infrequent that weak muscles and ligaments start to perform active, heavy work, while strong, large muscles are excluded from work and are inactive. The human body stops functioning as a well-differentiated whole, where all parts interact hierarchically and function harmonically, ensuring complete nerves conduction from periphery to the bran and back, and good blood flow.

As soon as the contracture is healed, several days later ganglions go away (the ganglions that are pressed out and surgically removed all over the world), pain stops, swelling subsides, flaccid, weak muscles liven up; auditory and muscle contact is restored. If the muscle is strained by contracture, tension of nerve fibers, wrapped around thereof (muscle spindle), decreases. There appears conductivity of nerve signals thereby from neural acoustic and emotional systems to fingers and of reverse signals from fingers to the brain. When contracture is healed, the musical performer’s arm becomes the speech organ again, the musician can freely express the imagery of the piece performed.

As is seen from the table, most arms diseases of musicians require long-term treatment, surgical treatment is often required.

Such diseases limit musicians’ working capacity and may require training for new occupation. The result is development of various reaction juxtapositions of neurotic character (asthenoneurotic syndrome, depression, etc.).

One of the serious problems a musician may face is possible hearing impairment.

Risk of hearing impairment as a result of exposure to noise of music depends on exposure intensity and duration. Average exposure time makes approximately 40 hours a week, however, many professional musicians may perform 60-80h a week. Hearing impairment is much more common among musicians than it might have been expected. 89% of professional musicians with osteomuscular traumas also have hearing impairments. Hearing diminishment in the range of 3-6KHz was found among them.

A musician’s work often implies concert tours. During such tours many musicians have to take long air flights which is very stressful. In addition to this a musician has no time for rest and relaxation as he or she has to spend a considerable amount of time rehearsing after arrival.

Jet lags, change of climate, etc. are additional stress factors. The organism does not have time to adjust to new living conditions.
